Everyday Support for Everyday Living
Our goals are to help individuals with intellectual, developmental disabilities and mental health challenges live more independently, stay connected to their communities, and reach their personal goals. We believe that these services address daily needs, promote skill-building, and enhance quality of life in meaningful, practical ways.
We tailor every service plan to the individual, working closely with families, caregivers, and support teams to ensure the right assistance is in place.
Dental Services (Den/De)
Comprehensive dental care, including preventive checkups, cleanings, and necessary treatments, to maintain oral health and overall well-being. Dental services are adapted to meet the unique needs of individuals with IDD.
Respite (Res/Reh)
Temporary relief for primary caregivers, ensuring that individuals receive quality care when their regular support person is unavailable. Respite can be provided in-home or out-of-home, allowing caregivers time to rest and recharge.
Therapies
Specialized therapy services to enhance independence, mobility, and communication:
Physical Therapy (PT) :
Improves strength, balance, and movement.
Occupational Therapy (OT) :
Builds skills for self-care, work, and community participation.
Speech Therapy (ST/SP) :
Improves communication, language, and swallowing abilities.
Employment Services
Programs designed to help individuals prepare for, find, and maintain meaningful employment.
Supported Employment (SE): Ongoing support for individuals once they’re hired.
Employment Assistance (EA): Job search help, resume building, and interview preparation.
Transportation (Tr/Trans)
Safe, reliable rides to workplaces, medical appointments, day programs, and community events—ensuring individuals can remain active and engaged.
Nursing Services
Skilled medical care from licensed professionals, including medication administration, health monitoring, and caregiver education.
Registered Nurse (RN)
Licensed Vocational Nurse (LVN)
Host Home / Companion Care (HHCC)
A warm, supportive living arrangement where trained caregivers welcome individuals into their homes and provide 24/7 assistance. This service fosters meaningful relationships, offers a stable home environment, and ensures access to daily care while promoting participation in family and community life.
Community First Choice (CFC)
Provides essential personal assistance with activities of daily living, including bathing, dressing, and mobility. CFC also offers habilitation training to build independent living skills, emergency response services, and support management. The goal here is to empower individuals to remain safely in their own homes.
Individualized Skills and Socialization (ISS)
Structured activities, both on-site and off-site, that help individuals build essential life skills, improve communication, and form social connections. ISS promotes personal growth and community engagement through hands-on learning and group participation.
(Formerly known as Day Habilitation)
Adaptive Aids (AA)
Specialized equipment, devices, and supplies that increase independence, mobility, and accessibility. This can include wheelchairs, communication devices, sensory aids, home safety equipment, and other tools that help individuals participate more fully in daily activities.
Minor Home Modifications (MHM)
MHM are physical changes to the home that make daily living safer and easier. Modifications such as installing wheelchair ramps, grab bars, widened doorways, or accessible bathroom fixtures are designed to meet the specific needs of the individual.
Behavioral Support Services (BES/PS)
Personalized strategies and interventions that address challenging behaviors and promote positive change. Our specialists work with individuals and their support teams to identify triggers, teach coping skills, and create behavior plans that improve daily life.
Residential Support Services (RSS)
Staff-supported living arrangements in shared homes for individuals who require daily assistance. RSS promotes independence while providing help with daily routines, health management, and community integration.
