Home and Environment Support
Our Minor Home Modification Maintenance program ensures that adaptive changes in the home remain safe and functional over time. Whether it’s repairing a ramp, adjusting grab bars, or updating other accessibility features, we keep home environments secure and supportive.
We also provide Pre-Enrollment Minor Home Modification (MHM) Assessments, helping families and individuals plan for needed changes before services begin. This includes a thorough evaluation of the home environment and recommendations to improve safety and accessibility.
Specialized Nursing Care
Some individuals require more intensive medical support. Our Nursing Specialized RN and Nursing Specialized LVN services offer expert care for complex health needs, ensuring that those with chronic or advanced medical conditions receive the highest quality support possible.
Health and Therapeutic Services
We provide a range of targeted health services, including Audiology for hearing assessments, Cognitive Rehabilitation Therapy to help individuals regain or improve mental skills, and Dietary Services to promote healthy nutrition and address specific dietary requirements.
Employment and Daily Living Support
For those preparing to join the workforce, our Employment Readiness services build skills, confidence, and workplace preparation. Our Social Work program connects individuals and families to community resources, advocacy, and counseling. We also offer Supervised Living arrangements for individuals who need structured support in a residential setting, promoting independence while ensuring safety and stability.
Transition Services

Our Transition Assistance Services help individuals move from one living situation to another, whether it’s transitioning from a facility to the community or moving to a new home environment. This includes both the service itself and associated Transition Assistance Services Fee coverage, ensuring that moves are smooth, supported, and successful.
Independent Living Skills (ILS)
For individuals ready to increase their self-reliance, our Independent Living Skills program focuses on building essential daily living abilities—such as cooking, budgeting, cleaning, and community navigation—so participants can live more independently.
